Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and skilled nursing tasks
• Assess and monitor resident conditions, record observations, and escalate changes to the care team
• Collaborate with nurses, physicians, and therapy staff to implement and update care plans
• Document treatments, vitals, and progress notes accurately and in a timely manner
• Educate residents and families on care plans, safety measures, and discharge instructions
• Maintain a safe, clean environment and follow infection control and facility protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license (New Mexico licensure preferred or eligible to obtain)
• Travel LPN experience preferred but motivated new travelers are welcome
• Basic Life Support (BLS) certification required or willingness to obtain promptly
• Demonstrated ability to work independently on overnight shifts and collaborate with a multidisciplinary team
• Current immunizations and ability to meet facility onboarding requirements

Education

• Graduate of an accredited LPN/LVN program; high school diploma or equivalent required

Knowledge & Skills

• Strong assessment skills and sound clinical judgment
• Medication administration and documentation proficiency
• Familiarity with long-term care and rehabilitation workflows (Rhb Ctr experience a plus)
• Excellent communication, time management, and patient education skills
• Comfortable with electronic health records and timely charting
