Roles & Responsibilities

• Provide compassionate direct patient care and observe changes in patient condition
• Administer medications and treatments accurately and document all care thoroughly
• Collaborate with nurses and interdisciplinary team members to develop and implement care plans
• Perform wound care, vital sign monitoring, and routine assessments with attention to detail
• Educate patients and families about care plans, medications, and follow up needs
• Maintain accurate charts and use the facility EHR to ensure continuity of care
• Advocate for patient needs and contribute to a positive, respectful care environment

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania or compactly eligible license
• BLS/CPR certification required or willing to obtain prior to start
• Previous experience in long term care, rehabilitation, or similar settings preferred
• Strong interpersonal and communication skills with a patient-centered mindset
• Ability to pass background checks and required facility onboarding

Education

• Graduate of an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Medication administration and safe handling practices
• Basic wound care and infection control protocols
• Proficient with charting and electronic health record systems
• Familiarity with skilled nursing and long term care workflows
• Time management and prioritization in a fast paced clinical environment
• Collaborative teamwork and clear communication with multidisciplinary staff
