Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care setting
• Administer medications accurately and document administration in the medical record
• Monitor and record vital signs, report changes in condition promptly to the team
• Collaborate with nurses, therapists and physicians to develop and follow care plans
• Perform wound care and other treatments per facility policies and standing orders
• Educate residents and families about care plans and health maintenance
• Maintain accurate, timely electronic charting and ensure regulatory compliance

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania required
• Current BLS/CPR certification required or ability to obtain prior to start
• Comfortable administering meds, performing assessments, and documenting in EMR
• Excellent communication skills and a team-first mindset
• Travel/contract experience is a plus but enthusiastic new travelers are welcome

Education

• Graduate of an accredited Practical Nursing program
• High school diploma or GED required; LPN program completion required

Knowledge & Skills

• Strong fundamentals in medication administration and resident assessment
• Proficient in documenting care in electronic medical records
• Knowledge of infection control and safety protocols
• Good time management and ability to prioritize tasks under typical long-term care demands
• Collaborative approach to working with interdisciplinary teams and families
