Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
- Administer medications and treatments accurately and document per facility policy
- Monitor resident conditions and communicate changes promptly to the care team
- Collaborate with RNs, CNAs, therapists, and families to create individualized care plans
- Assist with daily living activities while promoting dignity and independence
- Participate in shift handoffs and maintain clear, timely charting to support continuity of care
- Support infection control and safety protocols to protect residents and staff
Qualifications
- Current LPN license in Massachusetts or compact/state-appropriate licensure
- Completion of an accredited practical nursing program
- Current CPR/BLS certification preferred or willingness to obtain upon hire
- Comfortable administering medications and performing routine nursing procedures
- Strong communication skills and ability to work as part of a multidisciplinary team
- Previous long-term care or rehab experience is a plus
Education
- Graduate of an accredited Practical Nursing program (LPN)
- High school diploma or GED required
Knowledge & Skills
- Solid clinical assessment and basic nursing skills
- Medication administration and accurate documentation
- Familiarity with long-term care or skilled nursing workflows
- Time management and prioritization during busy 12-hour shifts
- Compassionate patient interaction and family communication
