Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in the Doctor's Rehab unit
- Administer medications and treatments accurately and on time
- Monitor patient status and report changes promptly to the care team
- Collaborate with nurses, therapists and physicians to implement individualized care plans
- Document patient care thoroughly and maintain accurate charts
- Support patient mobility and rehabilitation goals with safe techniques
- Educate patients and families about care plans, medications and follow-up
Qualifications
- Current LPN license in good standing (required)
- BLS (Basic Life Support) certification required; ACLS preferred if applicable
- Comfortable working in a rehabilitation/long-term care setting
- Strong communication skills and a team-first attitude
- Ability to pass facility background checks and meet onboarding requirements
Education
- LPN diploma or certificate from an accredited program
- For RN candidates (if applying as RN), ADN or BSN from an accredited program
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Familiarity with rehab nursing principles and mobility assistance
- Accurate clinical documentation and electronic charting
- Ability to prioritize tasks and respond to changing patient needs
- Strong interpersonal skills for working with patients, families and interdisciplinary teams
