Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the Doctor's Rehab unit
• Administer medications and treatments accurately and on time
• Monitor patient status and report changes promptly to the care team
• Collaborate with nurses, therapists and physicians to implement individualized care plans
• Document patient care thoroughly and maintain accurate charts
• Support patient mobility and rehabilitation goals with safe techniques
• Educate patients and families about care plans, medications and follow-up

Qualifications

• Current LPN license in good standing (required)
• BLS (Basic Life Support) certification required; ACLS preferred if applicable
• Comfortable working in a rehabilitation/long-term care setting
• Strong communication skills and a team-first attitude
• Ability to pass facility background checks and meet onboarding requirements

Education

• LPN diploma or certificate from an accredited program
• For RN candidates (if applying as RN), ADN or BSN from an accredited program

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Familiarity with rehab nursing principles and mobility assistance
• Accurate clinical documentation and electronic charting
• Ability to prioritize tasks and respond to changing patient needs
• Strong interpersonal skills for working with patients, families and interdisciplinary teams
