Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, making each day safer and more comfortable
- Administer medications and treatments accurately and on schedule
- Monitor patient status and report changes promptly to the care team
- Assist with activities of daily living while promoting resident dignity and independence
- Document clinical care thoroughly and communicate clearly with interdisciplinary staff
- Support admissions, discharges and transfers to ensure smooth transitions
Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of practice (required)
- Basic Life Support (BLS) or CPR certification (required or obtain upon hire)
- Previous LPN experience in long-term care or rehab settings preferred
- Strong clinical judgment, reliable attendance and a team-oriented mindset
- Ability to pass facility-required background and health screenings
Education
- Completion of an accredited LPN/Vocational Nursing program and high school diploma or equivalent
Knowledge & Skills
- Medication administration and basic IV/SC skills as applicable
- Wound care fundamentals and ability to follow care plans
- Resident assessment and vital sign monitoring
- Clear, compassionate communication with residents and families
- Electronic charting and documentation skills
- Time management and prioritization during busy 12-hour shifts
