Roles & Responsibilities

• Provide compassionate bedside care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and on time
• Perform and record vital signs and routine clinical assessments
• Document care clearly in the electronic health record for continuity
• Collaborate with RNs, providers, and interdisciplinary team members to support care plans
• Support resident comfort, safety, and dignity during overnight hours
• Respond to patient concerns and escalate changes in condition promptly
• Contribute to a positive, teamwork-driven environment that improves outcomes

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico required
• Basic Life Support (BLS) certification required or obtained upon hire
• Previous LPN experience in long-term care, rehab, or acute settings preferred
• Current immunizations and ability to meet facility health requirements
• Strong communication skills and a patient-centered attitude

Education

• Graduate of an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Medication administration and documentation expertise
• Skilled in routine nursing assessments and monitoring
• Comfortable using electronic health record systems
• Excellent clinical judgment and the ability to prioritize in busy shifts
• Strong teamwork and communication with interdisciplinary staff
• Basic wound care and ADL support experience
