Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for residents at Columbia Crest
• Assess patient condition, monitor vital signs, and implement nursing interventions
• Administer medications and treatments safely and document per policy
• Collaborate with the interdisciplinary team to create and update care plans
• Accurately document assessments, interventions, and patient responses in the chart
• Educate patients and families about care plans, medications, and follow-up
• Respond quickly and professionally to changes in patient condition and emergencies
• Support a positive, respectful environment that prioritizes resident dignity

Qualifications

• Active Registered Nurse or Licensed Practical Nurse license in good standing (RN preferred)
• Basic Life Support (BLS) certification required; additional certifications encouraged
• Comfortable working evening shift hours (2:00 PM - 10:30 PM)
• Strong clinical judgment, reliable documentation, and effective communication skills
• Previous experience in long-term care, skilled nursing, or similar preferred

Education

• Graduate of an accredited nursing program (LPN diploma or RN diploma/ADN/BSN as applicable)

Knowledge & Skills

• Strong medication administration and safety practices
• Skilled in patient assessment and care planning
• Competent documentation in electronic health records
• Ability to prioritize tasks in a fast-paced environment
• Clear, compassionate communication with residents, families, and coworkers
• Teamwork mindset and flexibility to support unit needs
