Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and document per facility policy
• Assess and monitor patient condition, reporting changes promptly to the care team
• Collaborate with nurses and interdisciplinary staff to build and follow individualized care plans
• Perform skilled nursing tasks and ensure adherence to infection control and safety standards
• Maintain clear, timely charting in the facility EMR to support continuity of care
• Support patient mobility, activities of daily living, and comfort measures with dignity

Qualifications

• Active Licensed Practical Nurse (LPN) license — Alabama license required or eligible
• Completion of an accredited Practical Nursing program
• Basic Life Support (BLS) certification — current or able to obtain prior to start
• Clinical experience in long-term care or skilled nursing preferred
• Strong communication skills and ability to work collaboratively with a care team
• Must be able to complete required facility onboarding and screenings

Education

• Practical Nursing diploma or equivalent from an accredited program

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment and vital signs monitoring
• Charting in electronic medical records (EMR) systems
• Infection prevention, wound care basics, and ADL support
• Time management and prioritization in a fast-paced clinical setting
• Teamwork, clear communication, and resident-centered care approach
