Roles & Responsibilities

• Provide compassionate, hands-on care to residents, ensuring dignity and safety at every step
• Administer medications accurately and document all care in the patient chart
• Perform routine assessments and promptly report changes in condition to the care team
• Assist with activities of daily living and promote patient independence
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Educate patients and families about treatment plans and follow-up care

Qualifications

• Valid Pennsylvania Licensed Practical Nurse (LPN) license in good standing
• Basic Life Support (BLS/CPR) certification required
• Current immunizations and TB screening per facility policy
• Clinical experience in long-term care or medical-surgical settings is a plus
• Strong communication skills and ability to work as part of a multidisciplinary team

Education

• Graduate of an accredited practical nursing program or equivalent

Knowledge & Skills

• Proficient in medication administration and documentation
• Solid knowledge of basic patient assessment and vital sign monitoring
• Familiarity with infection control practices and resident safety protocols
• Comfortable using electronic health records and charting accurately
• Good time management and the ability to prioritize patient needs
