Roles & Responsibilities

• Provide compassionate, high-quality bedside care to residents at Hillcrest Center
• Administer medications and treatments accurately and document per facility policies
• Monitor resident conditions and communicate changes promptly to the care team
• Assist with activities of daily living while encouraging independence and dignity
• Collaborate with nurses, therapists, and support staff to develop and follow care plans
• Maintain a safe, clean environment and adhere to infection control standards

Qualifications

• Active Licensed Practical Nurse (LPN) license or eligibility to obtain licensure in Pennsylvania
• Current BLS/CPR certification preferred or willingness to obtain upon hire
• Previous long-term care or skilled nursing experience is a plus
• Reliable, professional, and patient-centered approach to resident care
• Ability to pass facility background checks and meet credentialing requirements

Education

• Completion of an accredited LPN/LVN program

Knowledge & Skills

• Strong medication administration and documentation skills
• Knowledge of infection control practices and safety protocols
• Comfortable performing assessments and reporting changes in condition
• Good communication and teamwork skills with residents, families, and staff
• Basic familiarity with electronic medical records and charting
