Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and routine treatments
• Perform timely patient assessments and document changes in condition accurately
• Collaborate with RNs, physicians, and interdisciplinary team members to deliver coordinated care
• Support patient comfort and safety by following facility protocols and infection control practices
• Educate patients and families about medications, treatments, and post-discharge care
• Maintain clear, professional charting in the facility’s documentation system
• Assist with activities of daily living and support patient mobility as needed
• Contribute to a positive unit culture by communicating effectively and mentoring support staff when appropriate

Qualifications

• Current Licensed Practical Nurse (LPN) license — Pennsylvania license required
• Basic Life Support (BLS/CPR) certification required or obtainable prior to start
• Current immunizations and TB screening per facility policy
• Comfortable administering medications and performing routine nursing procedures
• Strong communication skills and the ability to work collaboratively in a team
• Previous experience in long-term care, skilled nursing, or acute care preferred but not mandatory

Education

• Graduate of an accredited Practical Nursing (LPN) program or equivalent vocational training

Knowledge & Skills

• Proficient in medication administration and accurate documentation
• Skilled in basic patient assessment and reporting clinical changes
• Knowledgeable about infection control, wound care basics, and vital sign monitoring
• Able to navigate electronic medical records and follow charting protocols
• Excellent interpersonal skills for patient- and family-centered care
