Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and monitoring
• Perform focused assessments and document patient status accurately and promptly
• Collaborate with RNs and interdisciplinary team members to implement care plans
• Support patient and family education to promote recovery and safe discharge
• Monitor and report changes in patient condition to ensure timely interventions
• Maintain charting, medication records, and compliance with facility policies

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Mexico (or compact state privileges where applicable)
• Minimum relevant clinical experience preferred; recent acute care or rehab experience a plus
• Current BLS (Basic Life Support) certification required
• Ability to pass background check, drug screen, and facility credentialing
• Strong communication skills and ability to work nights as part of a collaborative team

Education

• Diploma or certificate in Practical Nursing (LPN) from an accredited program
• State LPN licensure

Knowledge & Skills

• Medication administration and accurate documentation
• Wound care and basic nursing interventions
• Patient assessment and vital sign monitoring
• Electronic medical record (EMR) familiarity and charting accuracy
• Effective communication, teamwork, and time management
