Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at Pine Point Center
• Administer medications and treatments accurately and safely
• Perform thorough assessments and monitor changes in patient condition
• Document care clearly and timely in the facility electronic charting system
• Collaborate with the interdisciplinary care team to develop and update care plans
• Support activities of daily living and promote patient comfort and dignity
• Educate patients and families about care plans, medications and follow-up
• Participate in facility quality improvement and safety initiatives

Qualifications

• Active and unencumbered LPN or RN license (license appropriate for practice in the assignment state)
• Current BLS/CPR certification or willingness to obtain prior to start
• Professional, patient-focused attitude with strong communication skills
• Ability to pass required background check and drug screening
• Previous clinical experience in long-term care or similar setting preferred

Education

• LPN: Graduate of an accredited practical nursing program or equivalent
• RN: Graduate of an accredited nursing program (ADN, diploma or BSN)

Knowledge & Skills

• Medication administration and safe documentation practices
• Patient assessment and vital sign monitoring
• Familiarity with infection prevention and standard precautions
• Comfortable using electronic medical records and charting systems
• Strong teamwork, time management and patient advocacy skills
