Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents based on assessed needs
• Administer medications and treatments accurately and safely
• Monitor and document patient conditions, changes, and responses to interventions
• Collaborate with interdisciplinary team members to coordinate high-quality care
• Perform routine assessments and escalate concerns promptly to providers
• Support residents and families with education, discharge planning, and emotional support
• Adhere to infection control, safety protocols, and facility policies
• Contribute to a positive, team-focused work environment that values respect and dignity

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license appropriate for the state of practice
• Current Basic Life Support (BLS) certification preferred or required per facility policy
• Previous experience in long-term care, skilled nursing, or similar settings is a plus
• Strong clinical judgment, attention to detail, and reliable documentation skills
• Ability to work collaboratively on a multidisciplinary team and prioritize patient care

Education

• LPN: Graduate of an accredited Practical Nursing program or equivalent
• RN: Graduate of an accredited Associate or Bachelor of Science in Nursing program

Knowledge & Skills

• Clinical assessment and monitoring of adult patients
• Medication administration and accurate charting
• Wound care and basic restorative nursing skills
• Electronic medical record (EMR) documentation and charting
• Clear communication with patients, families, and care teams
• Time management, organization, and ability to handle a fast-paced environment
