Roles & Responsibilities
- Provide direct patient care with compassion and attention to detail
- Administer medications and treatments safely and according to policy
- Monitor patients and report changes in condition to the care team
- Document clinical observations and interventions accurately and promptly
- Support residents during activities of daily living and rehabilitation
- Collaborate with nurses, rehab staff and providers to deliver coordinated care
- Educate residents and families about care plans and promote comfort
Qualifications
- Required: Valid Licensed Practical Nurse (LPN) license in Massachusetts
- Comfortable working a 12-hour day shift schedule, 8:00 AM - 8:00 PM
- Strong communication skills and a team-oriented attitude
- Preferred: BLS/CPR certification and experience in rehab or long-term care
- Ability to pass background check and meet facility health requirements
Education
- Graduate of an accredited LPN training program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration and basic wound care
- Proficient with patient documentation and electronic charting
- Knowledge of infection control best practices
- Ability to prioritize tasks and respond effectively in a fast-paced setting
- Compassionate bedside manner and strong interpersonal skills
