Shift Type: Three 12-hour day shifts per week, 7:00 AM - 7:30 PM; schedule includes weekdays with an every-other-Sunday rotation and some holiday coverage (day shift)
Candidate Pay Rate: $33 per hour

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents at Elm Wood Center with professionalism and attention to detail
- Administer medications and treatments according to facility policies and physician orders
- Monitor and document resident conditions, reporting changes promptly to the care team
- Collaborate with nurses, therapists, and support staff to deliver coordinated care plans
- Support resident dignity and comfort through respectful communication and person-centered care
- Participate in shift handoffs and maintain accurate, timely documentation
Qualifications
- Active Licensed Practical Nurse (LPN) license in New Hampshire (required)
- Current BLS/CPR certification (required) and willingness to maintain mandatory training
- Clinical experience in long-term care, skilled nursing, or similar settings preferred
- Ability to work local schedule including every-other-Sunday rotation and some holidays
- Up-to-date immunizations and ability to pass facility health screening
Education
- Graduate of an accredited Practical Nursing program (LPN diploma/certificate)
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Comfortable with electronic health record documentation and bedside charting
- Excellent communication and teamwork skills with residents and interdisciplinary staff
- Reliable time management, ability to prioritize tasks during busy 12-hour shifts
- Compassionate approach to resident care and strong observational skills
