Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Elm Wood Center with professionalism and attention to detail
• Administer medications and treatments according to facility policies and physician orders
• Monitor and document resident conditions, reporting changes promptly to the care team
• Collaborate with nurses, therapists, and support staff to deliver coordinated care plans
• Support resident dignity and comfort through respectful communication and person-centered care
• Participate in shift handoffs and maintain accurate, timely documentation

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Hampshire (required)
• Current BLS/CPR certification (required) and willingness to maintain mandatory training
• Clinical experience in long-term care, skilled nursing, or similar settings preferred
• Ability to work local schedule including every-other-Sunday rotation and some holidays
• Up-to-date immunizations and ability to pass facility health screening

Education

• Graduate of an accredited Practical Nursing program (LPN diploma/certificate)

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Comfortable with electronic health record documentation and bedside charting
• Excellent communication and teamwork skills with residents and interdisciplinary staff
• Reliable time management, ability to prioritize tasks during busy 12-hour shifts
• Compassionate approach to resident care and strong observational skills
