Roles & Responsibilities

• Provide compassionate, hands-on care to residents including assistance with activities of daily living
• Monitor and record vital signs and communicate changes to the nursing team
• Support safe transfers and mobility, using proper body mechanics and equipment
• Maintain clear, accurate documentation and follow care plans
• Assist with meal service and feeding support as needed
• Promote a clean, safe environment by following infection control and safety protocols
• Collaborate with the interdisciplinary team to deliver person-centered care

Qualifications

• Active LNA / Licensed Nursing Assistant certification (VT or NH eligibility preferred)
• Proven experience providing resident care in a long-term care or similar setting preferred
• Must have reliable transportation to and from shift
• Ability to work an every-other-weekend commitment
• All requested time-off (RTO) must be approved prior to accepting assignment

Education

• Completion of an approved LNA training program and active LNA certification
• High school diploma or equivalent

Knowledge & Skills

• Strong knowledge of ADLs, infection control, and basic resident care
• Comfortable taking and recording vital signs and reporting changes
• Good communication skills with residents, families, and clinical staff
• Basic charting/documentation skills and ability to follow care plans
• Ability to work independently and as part of a team in a fast-paced environment
