Roles & Responsibilities

• Provide compassionate direct patient care, focusing on safety and dignity
• Administer medications and treatments accurately and document thoroughly
• Perform routine nursing assessments and monitor vital signs to catch changes early
• Collaborate with RNs, providers and the care team to develop and follow patient care plans
• Educate patients and families about care procedures and post-discharge instructions
• Maintain accurate electronic health record documentation and transfer information clearly at handoff

Qualifications

• Current LPN license in New Hampshire (or compact license where applicable)
• Graduate of an accredited practical nursing program
• Basic Life Support (BLS) certification required or willingness to obtain prior to start
• Active immunizations and ability to pass required background and health screenings
• Previous LPN experience preferred; strong clinical judgment and dependable attendance

Education

• Graduate of an accredited Practical Nursing program; LPN licensure required

Knowledge & Skills

• Skilled in medication administration and wound care basics
• Proficient with patient assessment and monitoring vital signs
• Comfortable with electronic health record documentation
• Strong communication and interpersonal skills for patient and team interactions
• Knowledge of infection control practices and safety procedures
• Ability to prioritize tasks in a fast-paced clinical environment
