Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in the Sandia Ridge Center area
• Administer medications and treatments safely and accurately according to facility policy
• Monitor and document patient vital signs, condition changes, and care provided in the chart
• Collaborate with RNs, providers, and interdisciplinary team members to support patient goals
• Educate residents and families about care plans, medications, and disease management
• Maintain accurate, timely documentation to support continuity of care and compliance
• Participate in shift handoffs and communicate concerns promptly to ensure patient safety

Qualifications

• Current LPN license in good standing (ability to obtain New Mexico LPN license if required)
• Basic Life Support (BLS) / CPR certification required or willing to obtain
• Current immunizations and ability to pass background and drug screening
• Comfortable working 12+ hour day shifts on Mon-Wed (6:00 AM - 6:30 PM)
• Prior LPN experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication skills and a patient-first attitude

Education

• Completion of an accredited LPN/LVVN program
• High school diploma or GED

Knowledge & Skills

• Skilled in medication administration and basic clinical procedures
• Proficient with vital sign assessment and patient observation
• Good documentation practices and familiarity with electronic health records
• Ability to prioritize tasks in a fast-paced care environment
• Team-oriented with strong interpersonal and communication skills
• Understanding of infection control and safety protocols
