Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Laconia Rehab Ctr
• Administer medications and treatments accurately, following facility policies
• Monitor resident conditions and communicate changes to the interdisciplinary team
• Document patient care clearly and promptly to support continuity and quality
• Support rehabilitation goals and assist with ADLs to help residents regain independence
• Collaborate with nursing staff and therapists to deliver person-centered care

Qualifications

• Current, active LPN license (able to practice in New Hampshire or appropriate compact/reciprocal authorization)
• Proven LPN experience in a long-term care, rehab or skilled nursing environment preferred
• Comfortable working 12+ hour day shifts and able to meet a 36-hour weekly guarantee
• Basic Life Support (BLS) certification required or able to obtain prior to start
• COVID-19 and other facility-required vaccinations or testing per policy

Education

• Completion of an accredited Practical Nursing program (LPN/LVN)

Knowledge & Skills

• Strong medication administration and documentation skills
• Familiarity with rehab/long-term care workflows and resident-centered care
• Ability to assess changes in condition and escalate appropriately
• Good communication skills with residents, families and interdisciplinary teams
• Time management and prioritization for busy 12.5-hour shifts
