Roles & Responsibilities
- Provide compassionate direct patient care and perform routine nursing assessments
- Administer medications and document treatments accurately and timely
- Monitor patients, report changes in condition, and escalate concerns to the care team
- Perform wound care and basic procedures within LPN scope of practice
- Collaborate with RNs, providers, and interdisciplinary staff to support patient goals
- Maintain clear, concise documentation in the patient record and support safe transitions of care
Qualifications
- Active LPN license (New Mexico) or eligible for New Mexico licensure
- Current BLS (Basic Life Support) certification preferred
- Comfortable working 12+ hour day shifts; reliability and strong attendance history
- Excellent communication skills and a team-player attitude
- Prior experience in long-term care, rehab, or similar clinical setting is a plus
Education
- Graduation from an accredited Practical Nursing/Vocational Nursing program or equivalent diploma/certificate
Knowledge & Skills
- Medication administration and reconciliation best practices
- Basic wound care and pressure injury prevention
- Patient assessment, vital signs, and recognition of clinical changes
- Electronic medical record documentation and charting accuracy
- Time management, prioritization, and ability to work in a fast-paced environment
- Strong interpersonal skills and patient-centered care approach
