Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the rehabilitation center
• Administer medications and treatments accurately and document care thoroughly
• Perform timely assessments and communicate changes in patient status to the care team
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Maintain clear, compliant clinical documentation and support quality improvement efforts
• Educate residents and families about care plans, medications, and safety practices

Qualifications

• Current Licensed Practical Nurse (LPN) license — active and in good standing
• Previous LPN experience in long-term care, rehab, or similar clinical setting preferred
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Strong documentation skills and comfort with electronic health records
• Excellent communication, teamwork, and resident-centered care focus

Education

• Completed accredited LPN/LVN program or equivalent vocational nursing diploma

Knowledge & Skills

• Skilled in medication administration, wound care basics, and ADL assistance
• Knowledge of infection control, safety protocols, and resident rights
• Ability to perform focused clinical assessments and report findings clearly
• Good time management, prioritization, and teamwork skills
