Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in the rehabilitation center
- Administer medications and treatments accurately and document care thoroughly
- Perform timely assessments and communicate changes in patient status to the care team
- Collaborate with interdisciplinary team members to create and follow individualized care plans
- Maintain clear, compliant clinical documentation and support quality improvement efforts
- Educate residents and families about care plans, medications, and safety practices
Qualifications
- Current Licensed Practical Nurse (LPN) license — active and in good standing
- Previous LPN experience in long-term care, rehab, or similar clinical setting preferred
- Basic Life Support (BLS) certification or willingness to obtain upon hire
- Strong documentation skills and comfort with electronic health records
- Excellent communication, teamwork, and resident-centered care focus
Education
- Completed accredited LPN/LVN program or equivalent vocational nursing diploma
Knowledge & Skills
- Skilled in medication administration, wound care basics, and ADL assistance
- Knowledge of infection control, safety protocols, and resident rights
- Ability to perform focused clinical assessments and report findings clearly
- Good time management, prioritization, and teamwork skills
