Roles & Responsibilities

• Provide compassionate bedside care and timely clinical assessments for assigned residents
• Administer medications and treatments accurately and document interventions promptly
• Collaborate with the interdisciplinary team to develop and update individualized care plans
• Monitor and report changes in patient condition to nursing leadership and providers
• Maintain thorough, compliant charting in the facility electronic medical record
• Support safe transfers, wound care, and infection prevention practices
• Educate residents and family members with clear, respectful communication

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice (New Mexico)
• Previous LPN experience in long-term care or skilled nursing preferred
• Basic Life Support (BLS/CPR) certification required or obtained upon hire
• Ability to pass background check and meet facility onboarding requirements
• Strong teamwork, reliability, and excellent communication skills

Education

• Completion of an accredited Practical Nursing program and high school diploma or GED

Knowledge & Skills

• Skilled in medication administration and clinical assessments
• Comfortable with wound care, IV support awareness, and basic procedures
• Solid documentation practices and familiarity with electronic medical records
• Infection control and resident safety best practices
• Compassionate patient communication and family support
