Roles & Responsibilities

• Administer medications and treatments safely and accurately to residents
• Monitor and document resident vital signs, condition changes, and care provided
• Collaborate with nurses, providers, and interdisciplinary team to deliver compassionate care
• Support resident activities of daily living and promote dignity and independence
• Report and escalate clinical concerns promptly to ensure resident safety
• Maintain accurate medical records and follow facility policies and procedures
• Participate in shift handoffs and communicate clear, concise care plans

Qualifications

• Current and active LPN license (New Jersey) required
• Basic Life Support (BLS) / CPR certification required
• Experience in long-term care or skilled nursing preferred
• Ability to work night shifts and every-other-weekend schedule
• Strong communication skills and ability to work as part of a multidisciplinary team

Education

• Completion of an accredited practical nursing program (LPN)

• Medication administration and documentation best practices
• Resident assessment and recognition of changes in condition
• Infection control and standard precautions
• Electronic medical records and bedside charting
• Time management and prioritization during busy night shifts
• Compassionate bedside manner and resident-centered care
