Roles & Responsibilities

• Provide high-quality direct patient care including ADL support and monitoring
• Administer medications and treatments accurately, following facility policies
• Perform routine assessments and document changes in patient condition
• Collaborate with RNs, physicians, and interdisciplinary team members to implement care plans
• Assist with wound care, dressing changes, and basic procedures as directed
• Educate patients and families on care plans, medications, and discharge instructions
• Maintain accurate, timely documentation in the patient record and report safety concerns

Qualifications

• Current Licensed Practical Nurse (LPN) license — New Mexico license preferred or eligibility to practice locally
• BLS/CPR certification required (or willing to obtain prior to start)
• Previous LPN experience in long-term care or similar setting preferred
• Strong communication and teamwork skills with compassionate patient focus
• Ability to pass facility background and health screening requirements

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Clinical knowledge of medication administration and basic nursing procedures
• Skilled in vital signs monitoring, patient assessments, and care documentation
• Familiarity with infection control practices and resident safety protocols
• Comfortable using electronic health records and completing accurate charting
• Time management and prioritization to handle a busy day shift
• Collaborative approach to working with nursing teams and ancillary staff
