Roles & Responsibilities

• Provide compassionate direct patient care to residents at Las Palomas Ctr, ensuring dignity and comfort
• Administer medications and treatments accurately and document all care thoroughly
• Monitor patient status and promptly report changes to the interdisciplinary team
• Collaborate with nurses, CNAs, therapists and providers to implement individualized care plans
• Educate patients and families on care routines, medications and safety measures
• Support smooth transitions of care during admissions, transfers and discharges

Qualifications

• Active LPN license in New Mexico or ability to obtain state LPN license prior to start
• Current BLS (CPR) certification required
• Valid background check and immunizations as required by facility
• Previous experience in long-term care, skilled nursing, or related setting preferred (1+ year preferred)
• Strong communication, time management and documentation skills

Education

• Graduate of an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Solid knowledge of medication administration and charting best practices
• Comfort working with geriatric populations and chronic care needs
• Ability to triage changes in condition and escalate appropriately
• Familiarity with electronic medical records and standard clinical workflows
• Team-oriented mindset with strong bedside manner
