Roles & Responsibilities

• Provide compassionate, person-centered care by assisting residents with activities of daily living (bathing, dressing, mobility, toileting)
• Observe and report changes in resident condition to nursing staff promptly to ensure timely interventions
• Document care accurately and promptly in PointClickCare (PCC) to maintain clear resident records
• Support safe transfers, ambulation, and fall prevention practices to protect residents
• Assist with meal service and feeding while monitoring dietary needs and preferences
• Follow individualized care plans and collaborate with the interdisciplinary team to meet resident goals
• Maintain infection control practices and a clean, safe environment for residents and staff

Qualifications

• Current LNA certification/licensure in the state of Vermont (or ability to obtain prior to start)
• Minimum experience working as a nursing assistant in a long-term care or skilled nursing setting preferred
• Required: PointClickCare (PCC) experience — must have hands-on PCC documentation experience
• Current BLS/CPR certification required
• Ability to pass background checks, health screenings, and provide immunization records
• Reliable, professional, and able to commit to every other weekend and some holiday coverage

Education

• High school diploma or GED and current LNA certification

Knowledge & Skills

• Proficient with PointClickCare (PCC) electronic health record documentation
• Strong knowledge of resident ADL support, basic nursing tasks, and safety protocols
• Good communication skills for interacting with residents, families, and clinical staff
• Time management and ability to prioritize tasks during busy evening shifts
• Basic understanding of infection control, bathing, dressing, and feeding techniques
• Compassionate bedside manner and ability to work in a team-oriented environment
