Roles & Responsibilities
- Provide compassionate direct nursing care to residents/clients during assigned shifts
- Administer medications and treatments accurately and document per policy
- Perform thorough assessments and report changes in condition to the care team
- Collaborate with RNs and interdisciplinary team to implement individualized care plans
- Maintain clear, timely charting in the facility electronic health record
- Support a safe, therapeutic environment through attentive observation and interventions
- Educate residents and families on care plans, medications and safety as needed
Qualifications
- Current Licensed Practical Nurse (LPN) license valid in New Mexico
- Basic Life Support (BLS) certification required or obtain upon hire
- Comfortable with medication administration and routine nursing procedures
- Ability to work Mon/Wed/Fri 7:00 AM - 7:30 PM reliably
- Previous experience in post-acute, long-term care or behavioral health is a plus
- Strong communication skills and a team-oriented mindset
Education
- LPN diploma or certificate from an accredited nursing program
- Current New Mexico LPN license
Knowledge & Skills
- Medication administration and documentation accuracy
- Patient assessment and reporting clinical changes
- Basic wound care and ADL assistance
- Familiarity with electronic health records and nursing documentation
- Time management and the ability to prioritize during a 12+ hour day
- Professional, empathetic communication with patients, families and staff
