Roles & Responsibilities

• Provide direct patient care with compassion and clinical accuracy—medication administration, wound care, and monitoring vital signs
• Collaborate with interdisciplinary team members to create and update individualized care plans
• Document patient status and interventions clearly and promptly in the electronic health record
• Communicate effectively with patients and families to educate, reassure, and involve them in care decisions
• Adhere to facility policies and infection control standards to keep patients and staff safe
• Support smooth transitions of care by coordinating with PCC and LTC resources when needed

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Mexico (required)
• Previous experience in PCC and/or long-term care preferred
• Current BLS/CPR certification (or willingness to obtain)
• Up-to-date immunizations and ability to pass background and drug screening
• Local candidate preferred (LOCAL ONLY) with reliable commute; Prefer No RTO
• Strong clinical judgment, good communication skills, and a patient-centered attitude

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED required

Knowledge & Skills

• Skilled in medication administration and basic nursing assessments
• Comfortable with electronic charting and documentation
• Strong observational skills for changes in patient condition
• Good teamwork and communication with clinical staff and leadership
• Familiarity with PCC and LTC workflows is a plus
