Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents as a Licensed Practical Nurse
• Administer medications and document medication records accurately and on time
• Perform routine assessments and promptly report changes in patient condition
• Collaborate with RNs, CNAs, and interdisciplinary team members to deliver coordinated care
• Assist with wound care, dressing changes, and other skilled nursing tasks within LPN scope
• Maintain clear, timely charting in the facility’s documentation system
• Support resident activities of daily living while promoting dignity and independence

Qualifications

• Current Licensed Practical Nurse (LPN) license in Vermont or ability to obtain promptly (LOCAL ONLY)
• Basic Life Support (BLS/CPR) certification required
• Previous experience in long-term care, rehab, or medical-surgical settings is preferred
• Comfort working night shifts and delivering dependable, patient-centered care
• Ability to pass facility background checks and meet onboarding requirements

Education

• Completion of an accredited practical nursing program; LPN diploma or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Care planning and basic patient assessment abilities
• Effective communication and teamwork with clinical staff and families
• Time management and prioritization during busy night shifts
• Familiarity with infection control practices and resident safety protocols
• Comfort using electronic health records and paper charting as required
