Candidate Pay Rate: $33 per hour

Roles & Responsibilities

• Provide compassionate, hands-on nursing care for assigned residents in accordance with care plans
• Administer medications and treatments accurately and document per facility policy
• Perform thorough assessments and escalate changes in condition to the RN or provider
• Collaborate with nursing staff, therapists, and physicians to deliver coordinated care
• Support resident comfort and dignity through personalized care and patient education
• Accurately chart assessments, interventions, and outcomes in the electronic record
• Participate in shift handoffs and interdisciplinary team meetings to ensure continuity of care

Qualifications

• Licensed Practical Nurse (LPN) — active and unencumbered licensure required
• BLS (Basic Life Support) certification required; ACLS or other certifications a plus
• Previous LPN experience in long-term care, rehab, or similar setting preferred (1+ year recommended)
• Ability to work the posted rotating biweekly schedule including weekends
• Current immunizations and ability to pass facility-required background checks

Education

• Graduate of an accredited Practical Nursing program
• Current LPN licensure

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and reporting changes in condition
• Familiarity with electronic health record charting
• Strong communication and teamwork skills
• Basic wound care, ADL support, and restorative nursing principles
