Roles & Responsibilities
- Provide compassionate, patient-centered care as a Licensed Practical Nurse on the unit
- Administer medications and treatments accurately according to facility policy
- Monitor and document patient status, reporting changes to the interdisciplinary team
- Support residents with activities of daily living while promoting dignity and independence
- Collaborate with nurses and clinical staff to implement effective care plans
- Maintain accurate, timely charting and ensure compliance with regulatory standards
- Participate in shift handoffs and communicate critical information clearly
Qualifications
- Active Licensed Practical Nurse (LPN) license — eligible to practice in New Hampshire preferred
- Current BLS/CPR certification or willingness to obtain upon hire
- Comfortable working a day shift schedule with consistent early start time
- Strong interpersonal and communication skills; team player attitude
- Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
Education
- Graduate of an accredited Practical/Vocational Nursing program
- State LPN licensure required
Knowledge & Skills
- Medication administration and basic IV/technical skills applicable to LPN scope
- Patient assessment, vital signs monitoring, and charting best practices
- Familiarity with long-term care routines and resident-centered care
- Time management, prioritization, and the ability to work efficiently under shift demands
- Positive communication with patients, families, and interdisciplinary teams
