Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to post-acute residents
- Administer medications and treatments accurately and document in the medical record
- Perform thorough assessments and escalate changes in condition to the care team
- Collaborate with interdisciplinary team members to create and implement care plans
- Educate patients and families about care plans, medications, and discharge instructions
- Maintain compliance with facility policies, safety protocols, and infection control practices
Qualifications
- Current, unencumbered Registered Nurse license for Pennsylvania or compact RN license valid for practice
- Basic Life Support (BLS) certification required; additional certifications encouraged
- Previous experience in post-acute, skilled nursing, or long-term care settings preferred
- Strong clinical assessment skills, sound judgment, and the ability to prioritize care
- Reliable, professional, and team-oriented approach to patient care
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Medication administration and IV site assessment
- Wound care basics and pressure injury prevention
- Effective documentation in electronic medical records
- Clear communication with patients, families, and the healthcare team
- Time management and prioritization in a fast-paced post-acute environment
