Roles & Responsibilities

• Provide compassionate direct patient care and monitor patient status during overnight hours
• Administer medications and treatments accurately and safely
• Collaborate with the interdisciplinary team to coordinate care plans
• Document patient progress, interventions, and responses thoroughly and timely
• Educate patients and families on care plans and discharge instructions
• Maintain a safe, clean, and therapeutic environment for patients
• Participate in quality improvement initiatives and contribute clinical insights

Qualifications

• Active Licensed Practical Nurse (LPN) license or T-LPN classification as required
• Current BLS/CPR certification preferred or willing to obtain
• Previous LPN experience in long-term care, rehabilitation, or similar settings preferred
• Ability to work scheduled overnight shifts (Sunday-Tuesday) and provide reliable coverage
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited LPN/Vocational Nursing program or equivalent

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Skilled at patient assessment and recognizing changes in condition
• Comfortable with electronic documentation systems
• Good time management and ability to prioritize in a fast-paced environment
• Collaborative approach to working with nurses, therapists, and support staff
