Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living (ADLs) such as bathing, dressing, and mobility
• Monitor and record vital signs and report any changes in condition to the clinical team promptly
• Assist with safe transfers and positioning to promote resident comfort and prevent injury
• Support individualized care plans by following instructions from nurses and rehabilitation staff
• Maintain accurate documentation of resident care, observations, and interventions
• Collaborate with the care team to deliver a respectful, dignified experience for every resident

Qualifications

• Current Certified Nursing Assistant (CNA) certification — required
• Eligible for or holding any state-required CNA registration for New Mexico
• Comfortable working 12+ hour day shifts (6:00 AM - 6:30 PM) on assigned days
• Strong communication skills and the ability to work as part of a multidisciplinary team
• Preferred: prior experience in rehab, long-term care, or similar settings
• Preferred: current CPR/BLS certification; ability to complete background check and drug screen

Education

• High school diploma or GED
• Completion of an approved CNA training program and active CNA certification

Knowledge & Skills

• Knowledge of basic nursing care and resident-centered practices
• Skill in assisting with ADLs, transfers, and mobility support
• Familiarity with vital sign measurement and documentation
• Ability to follow care plans and report clinical changes accurately
• Strong interpersonal skills, patience, and cultural sensitivity
• Basic charting and electronic documentation skills
