Roles & Responsibilities
- Provide compassionate direct patient care to residents according to care plans
- Administer medications and treatments safely and accurately
- Monitor and document patient status, reporting changes to the care team
- Collaborate with RNs and interdisciplinary staff to support resident well-being
- Perform admission, transfer and discharge procedures when needed
- Support patient comfort and dignity through respectful, person-centered care
Qualifications
- Current Licensed Practical Nurse (LPN) license required
- CPR/BLS certification required
- Previous experience in long-term care or similar setting preferred
- Comfortable with medication administration, wound care basics and documentation
- Ability to pass background checks and meet facility onboarding requirements
Education
- LPN diploma or equivalent practical nursing program completion
Knowledge & Skills
- Strong clinical assessment and monitoring skills
- Accurate medication administration and charting
- Good communication and teamwork with interdisciplinary staff
- Time management for efficient 12-hour shift workflow
- Basic infection control and safety practices
