Roles & Responsibilities

• Provide compassionate bedside nursing care to residents, focused on safety and dignity
• Administer medications and treatments according to physician orders and facility policies
• Perform regular assessments and promptly report changes in patient condition
• Document care accurately and timely in the electronic medical record
• Collaborate with the interdisciplinary team to develop and follow individualized care plans
• Support residents with activities of daily living and promote a positive living environment
• Assist with admissions, transfers, and discharges to ensure smooth transitions

Qualifications

• Current Licensed Practical Nurse (LPN) license valid for practice in the assignment state
• Basic Life Support (BLS/CPR) certification required
• Prior LPN experience in long-term care or skilled nursing preferred
• Ability to pass background check, drug screen, and meet facility health requirements
• Strong clinical judgment, reliable attendance, and a team-first attitude

Education

• Graduate of an accredited practical nursing program (or equivalent)

Knowledge & Skills

• Solid knowledge of medication administration and documentation standards
• Skilled in resident assessment, vital signs, and basic wound care
• Comfortable using electronic medical records for charting
• Excellent communication and interpersonal skills with residents and families
• Ability to prioritize tasks in a fast-paced long-term care environment
