Roles & Responsibilities

• Provide compassionate, hands-on personal care to residents including bathing, dressing, grooming and toileting
• Assist with activities of daily living and support residents’ independence wherever possible
• Monitor residents’ condition and promptly report changes to nursing staff to ensure safe, timely care
• Accurately document care, vitals and observations in the facility record to maintain clear communication
• Collaborate with the care team to implement individualized care plans and promote a positive living environment
• Support mealtime assistance and safe feeding practices, including following special diet instructions

Qualifications

• Active Licensed Nursing Assistant (LNA) license required; T-LNA credentials accepted
• Ability to obtain or hold required state licensure for New Hampshire
• CPR/BLS certification preferred or willingness to obtain
• Comfortable assisting with mobility and physical care; ability to pass background check and drug screen
• Strong communication skills and a teamwork mindset—patients and coworkers will thank you

Education

• High school diploma or equivalent and successful completion of an approved LNA program

Knowledge & Skills

• Skilled in activities of daily living (ADLs) and personal care delivery
• Knowledge of infection control and safe patient handling practices
• Ability to take and record vital signs accurately
• Clear, compassionate communication with residents, families and the care team
• Basic charting and documentation skills for resident records
