Roles & Responsibilities
- Provide compassionate, hands-on personal care to residents including bathing, dressing, grooming and toileting
- Assist with activities of daily living and support residents’ independence wherever possible
- Monitor residents’ condition and promptly report changes to nursing staff to ensure safe, timely care
- Accurately document care, vitals and observations in the facility record to maintain clear communication
- Collaborate with the care team to implement individualized care plans and promote a positive living environment
- Support mealtime assistance and safe feeding practices, including following special diet instructions
Qualifications
- Active Licensed Nursing Assistant (LNA) license required; T-LNA credentials accepted
- Ability to obtain or hold required state licensure for New Hampshire
- CPR/BLS certification preferred or willingness to obtain
- Comfortable assisting with mobility and physical care; ability to pass background check and drug screen
- Strong communication skills and a teamwork mindset—patients and coworkers will thank you
Education
- High school diploma or equivalent and successful completion of an approved LNA program
Knowledge & Skills
- Skilled in activities of daily living (ADLs) and personal care delivery
- Knowledge of infection control and safe patient handling practices
- Ability to take and record vital signs accurately
- Clear, compassionate communication with residents, families and the care team
- Basic charting and documentation skills for resident records
