Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents according to care plans
- Administer medications and treatments safely and document accurately
- Monitor residents' conditions, report changes, and escalate concerns promptly
- Collaborate with the care team to implement individualized care plans
- Assist with ADLs and promote resident dignity, independence, and comfort
- Maintain accurate and timely clinical documentation and handoffs
- Participate in quality improvement and facility safety initiatives
Qualifications
- Active Licensed Practical Nurse (LPN) license valid in Vermont
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Ability to pass background check and drug screening per facility policy
- Prior LPN experience in long-term care or similar setting preferred
- Strong communication skills and a collaborative, patient-centered approach
Education
- Graduate of an accredited LPN/LVN or vocational nursing program
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic wound care and infection control principles
- Familiarity with electronic health records or clinical documentation systems
- Ability to assess basic patient conditions and report changes effectively
- Time management and prioritization in a fast-paced care environment
