Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in the facility
• Administer medications and treatments accurately and document thoroughly
• Monitor and record vital signs, reporting changes promptly to the care team
• Collaborate with RNs, CNAs, and interdisciplinary staff to support care plans
• Assess patients regularly and contribute to care plan updates and discharge planning
• Maintain accurate, timely charting in the facility’s documentation system
• Promote a safe environment by following infection control and safety protocols

Qualifications

• Valid LPN license in New Hampshire (required)
• Completion of an accredited practical nursing program
• Current BLS (Basic Life Support) certification or willingness to obtain
• Previous LPN experience in long-term care or skilled nursing preferred
• Excellent communication skills and ability to work as part of a collaborative team
• Ability to pass facility background checks and required health screenings

Education

• Graduate of an accredited LPN/LVN program

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable with clinical assessments and basic wound care
• Familiarity with long-term care workflows and resident-centered care
• Proficient in accurate vital sign measurement and reporting
• Good time management and ability to prioritize in a busy setting
• Professional bedside manner and effective communication with patients and families
