Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients according to care plans
• Administer medications and treatments accurately and safely
• Perform regular assessments and monitor patient vital signs, reporting changes promptly
• Document care and patient progress clearly and professionally in the chart
• Collaborate with nurses, CNAs, therapists, and providers to deliver coordinated care
• Educate patients and families about care instructions and discharge planning
• Support infection control, safety, and facility policies to protect patients and staff
• Contribute to a positive patient experience through empathy and professionalism

Qualifications

• Active Licensed Practical Nurse (LPN) license (required)
• Current BLS/CPR certification or ability to obtain prior to start
• Up-to-date immunizations and ability to pass required health screenings
• Ability to pass background check and any facility-required screenings
• Local candidates only as noted in the order
• Previous LPN experience in long-term care or similar setting is preferred

Education

• Graduate of an accredited LPN/LVN program (diploma or certificate)

Knowledge & Skills

• Strong medication administration and documentation skills
• Basic patient assessment and clinical observation
• Familiarity with infection control and safety practices
• Good communication and teamwork skills with patients and staff
• Time management and ability to prioritize in a busy clinical setting
• Comfortable using electronic medical records and charting systems
