Roles & Responsibilities

• Provide compassionate, hands-on care to residents including assistance with activities of daily living
• Monitor and document vital signs, behavior, and any changes in condition promptly
• Support residents with mobility, meals, hygiene, and comfort in a respectful manner
• Collaborate with nurses and interdisciplinary team to follow care plans and ensure continuity
• Maintain a safe, clean environment and follow infection control and safety protocols
• Communicate effectively with residents and families to promote dignity and person-centered care

Qualifications

• Current LNA license (VT or NH as indicated in job title)
• Must have reliable personal transportation and own car; public transport is not reliable
• Ability to pass required background checks and health screenings
• Strong communication skills and a team-first attitude
• Previous experience in long-term care or rehabilitation settings is a plus

Education

• Completion of an approved Nursing Assistant program or equivalent certification
• High school diploma or GED preferred

Knowledge & Skills

• Comfortable assisting with activities of daily living and personal care
• Basic vital signs monitoring and documentation skills
• Familiarity with infection control practices and resident safety procedures
• Good interpersonal skills for resident and family interactions
• Reliable time management and the ability to follow care plans accurately
