Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and safely following facility policy
• Monitor and document patient status, vital signs, and changes in condition in a timely manner
• Collaborate with nurses and interdisciplinary team members to coordinate resident care
• Support residents with activities of daily living while promoting dignity and independence
• Participate in shift handoffs and maintain clear, professional communication with the care team
• Follow infection control practices and facility procedures to ensure resident safety

Qualifications

• Active Licensed Practical Nurse (LPN) license — ready to practice
• Current Basic Life Support (BLS) certification or able to obtain upon hire
• Comfortable working overnight shifts and providing hands-on resident care
• Strong communication skills and the ability to work well on a multidisciplinary team
• Previous long-term care or skilled nursing experience preferred but not required

Education

• Completion of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Skilled in resident assessment, basic wound care, and vital sign monitoring
• Familiarity with electronic health records or willingness to learn quickly
• Good time management, prioritized care delivery, and clear charting habits
• Ability to follow care plans and communicate changes to RN leadership
