Roles & Responsibilities

• Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse
• Administer medications and treatments safely and accurately
• Monitor residents vital signs and condition, reporting changes promptly
• Document care and interventions clearly in the clinical record
• Collaborate with nursing team and interdisciplinary staff to support resident goals
• Assist with daily living activities and promote dignity and comfort for residents
• Support admission, transfer and discharge processes as needed

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing required
• Completion of an accredited practical nursing program
• CPR/BLS certification required or obtained upon hire
• Previous experience in long term care or similar clinical setting is preferred
• Ability to work the 3:00 PM - 11:30 PM evening shift and participate in every-other-weekend coverage
• Strong communication, teamwork and time-management skills

Education

• Graduate of an accredited Practical Nursing program
• High school diploma or equivalent
• Current LPN license

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and monitoring skills
• Familiarity with infection control and safety procedures
• Comfortable working with interdisciplinary teams
• Professional bedside manner and resident-centered communication
• Experience with electronic medical records is a plus
