Roles & Responsibilities

• Provide compassionate bedside nursing care to residents in a rehab/long-term care setting
• Administer medications and treatments accurately and on schedule
• Document resident condition, interventions, and responses in the medical record promptly
• Monitor vital signs and report clinical changes to the care team
• Collaborate with RNs, therapists, and interdisciplinary staff to support individualized care plans
• Educate residents and families about care plans, medications, and safety measures
• Maintain a safe environment and escalate clinical concerns when necessary

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico
• Current BLS/CPR certification (or obtain upon hire)
• Ability to work scheduled nights: Monday, Tuesday, Wednesday (overnight shift)
• Local candidate only (per job note) — must be able to commute to the facility
• Professional, patient-centered approach and strong communication skills
• Previous experience in rehab, long-term care, or similar setting is preferred

Education

• LPN diploma or vocational nursing certificate required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and restorative nursing skills
• Skilled in resident assessment and recognizing clinical changes
• Familiarity with electronic health records and accurate charting
• Strong teamwork, time management, and patient advocacy
• Ability to follow care plans and work within interdisciplinary teams
