Roles & Responsibilities
- Provide compassionate, patient-centered bedside care to residents in the assigned unit
- Administer medications and treatments accurately and document per policy
- Perform nursing assessments and communicate changes in condition to the care team
- Collaborate with nurses, therapists, and support staff to create individualized care plans
- Maintain accurate charts and electronic medical records with timely documentation
- Educate patients and families about care plans, medications, and follow-up needs
- Assist with admissions, discharges, and transfers to ensure smooth transitions
Qualifications
- Licensed Practical Nurse (LPN) - active license required
- Current BLS/CPR certification (or ability to obtain upon hire)
- Comfortable working Monday-Wednesday, 6:00 AM - 6:30 PM for the 12-week assignment
- Previous experience in long-term care, rehab, or similar settings preferred
- Local candidates only (per facility note)
Education
- Graduate of an accredited Practical Nursing (LPN) or vocational nursing program
Knowledge & Skills
- Strong medication administration and documentation skills
- Patient assessment and basic wound care knowledge
- Familiarity with electronic medical records and charting best practices
- Excellent communication and teamwork skills
- Ability to prioritize tasks and work efficiently during 12+ hour shifts
