Roles & Responsibilities

• Perform thorough bedside swallow evaluations and dysphagia assessments to keep patients safe and nourished
• Develop and implement individualized swallowing therapy and language/communication treatment plans
• Conduct cognitive assessments and tailor interventions to patient needs
• Lead discharge planning evaluations to ensure safe transitions and continuity of care
• Provide clear, compassionate patient and family education on dysphagia precautions and modified diets
• Document timely, accurate clinical notes in EPIC and maintain excellent charting standards
• Collaborate closely with PT, OT, nursing and rehab aides as part of a highly engaged interdisciplinary team
• Participate in department orientation and ongoing clinical learning to support best practices

Qualifications

• Active Speech-Language Pathologist license (facility requires license to submit)
• BLS (Basic Life Support) certification — required
• Minimum 1 year of clinical experience (acute inpatient experience preferred)
• Comfort with bedside swallow evaluations, dysphagia management and swallowing therapy
• Experience with EPIC or similar electronic medical records for accurate charting
• Ability to work rotating weekends and holiday coverage when scheduled
• Strong communication skills and a team-focused approach

Knowledge & Skills

• Expertise in dysphagia assessment and management, including bedside swallow evaluations
• Skills in language, communication and cognitive assessment and therapy
• Familiarity with acute inpatient diagnoses: neurological, cardiac, critical care, oncology, pulmonary, orthopedics
• Proficiency with EPIC charting and timely clinical documentation
• Ability to educate patients and families about diet modifications and safety precautions
• Collaborative practice within interdisciplinary rehab teams
