Roles & Responsibilities

• Assess patients’ physical function and mobility to establish baseline needs
• Develop individualized treatment plans that promote recovery and independence
• Implement therapeutic interventions including gait training, strengthening, and balance exercises
• Document care thoroughly and accurately in PointClickCare
• Collaborate with physicians, nurses, and the interdisciplinary team to coordinate care
• Educate patients and families on home programs, safety, and fall prevention
• Contribute to discharge planning and smooth transitions to next-level care
• Maintain professional standards, complete orientation, and participate in unit-based initiatives

Qualifications

• Current, unrestricted Physical Therapist license (able to submit for the position)
• Minimum 2 years of Physical Therapy experience (acute care experience preferred)
• BLS (Basic Life Support) certification required
• Familiarity with PointClickCare or similar electronic charting systems
• Comfortable working in an acute care environment and collaborating with a multidisciplinary team
• Strong clinical reasoning, communication, and documentation skills

Knowledge & Skills

• Acute care physical therapy practices and protocols
• Patient assessment and clinical reasoning for medically complex patients
• Therapeutic exercise, gait training, transfers, and mobility training
• Skilled documentation and charting (PointClickCare experience a plus)
• Infection control and safety procedures in a hospital setting
• Effective patient & family education and discharge planning
