Roles & Responsibilities
- Perform thorough patient evaluations to create individualized OT plans of care
- Complete revisits and follow-up visits (average four visits per day) to monitor progress
- Document accurately and timely in Cerner to ensure continuity of care and billing compliance
- Coordinate with home health supervisors for mileage/timesheet approvals and sign-offs
- Provide compassionate, patient-centered care in the home setting to improve function and safety
- Collaborate with interdisciplinary team members to optimize patient outcomes
- Manage your caseload and travel within the coverage area efficiently while maintaining clinical quality
Qualifications
- Active California OT license (required)
- Minimum 2 years' experience in a healthcare setting
- BLS certification (required)
- NPI number (required)
- Valid driver's license and professional automobile insurance (required for home health)
- Two supervisory references at submittal (one within 12 months and one within 3 years worked)
- Willingness to float first within the defined coverage area
- COTA candidates with home health experience will be considered only if no OT applicants are available
Knowledge & Skills
- Strong proficiency with Cerner charting and electronic documentation
- Skilled in patient evaluations, creating treatment plans, and conducting revisits
- Excellent time management and travel planning for home health routes
- Ability to work independently in patients' homes with strong clinical judgment
- Clear communication and collaboration with supervisors and interdisciplinary teams
- Attention to detail for accurate mileage reporting and timesheet submission
- Familiarity with post-acute/home health care workflows and regulations
